In ASSIST - Fertility, we store our clients' data in accordance with the Data Protection Act. Our Therapy notes are held securely and confidentially for six years post-therapy, after which they are destroyed. These records do not form part of any other record system within the National Healthcare services.
We also store basic information about each of our clients on a secure and confidential database for future references. These are kept separate to any other record system. This information is used to compile aggregate data and monitor our service as a whole. Generally, no information that can be traced back to our clients will be released to any other agency without our clients' explicit permission. There are some exceptional circumstances where this confidentiality policy will be breached. For example, we have duty of care to inform your GP if we are concerned that there is serious risk of harm to yourself or others. For more information on this, please contact us.
